What to do when you are injured on the job:

1. An employee must report any injury to their supervisor during the shift that the injury occurs or the employee may lose the right to benefits.
2. Employee must submit 1st Report of Incident within 24 hours of the injury.  The Supervisor of the employee must submit the Accident Investigation Report within 24 hours of injury to employee.
3. If treatment is needed, please report to:

If after hours or the weekend visit an Insta-Care or hospital if necessary.

4. Inform provider that it is a work related injury.  This will ensure that your visit is properly billed and not delayed.
